Hair loss: stress or hereditary? The difference explained

Is your hair suddenly falling out much more than usual? You’ll want to know if it will grow back. That depends on the cause. Hair loss caused by stress, also known as telogen effluvium, usually occurs suddenly and is spread across the entire scalp. This form is often temporary. Hereditary hair loss develops gradually at the hairline, temples, crown, or top of the head and does not resolve on its own. What is stress-related hair loss?

With telogen effluvium, many hairs enter the resting phase simultaneously over a short period. A few months later, these hairs shed. This causes you to temporarily lose much more hair than normal.

The shedding is usually spread across the entire scalp, including the crown. Generally, no distinct bald patches form. Your hair simply feels less full overall, and you notice more hair in the shower, on your brush, or on your pillow.

Possible triggers include:

  • a prolonged or intense period of stress
  • illness or high fever
  • surgery
  • rapid weight loss
  • a severe nutritional deficiency
  • a change in medication

When does stress-related hair loss begin?

Hair loss due to stress usually does not start immediately after the event. There is often a two- to four-month gap between the trigger and the moment the hair begins to fall out.

As a result, the connection is not always immediately clear. The stressful period, illness, or surgery may already be over by the time the hair loss begins.

Therefore, look back at what has happened over the past few months.

Does hair grow back after stress?

Usually, yes. With telogen effluvium, the hair follicles themselves are generally not damaged. Only the timing of the natural hair cycle is temporarily disrupted.

Once the underlying cause is addressed, normal hair growth can resume. It may take several months for shedding to decrease and for new hair to become visible.

What is hereditary hair loss?

In cases of hereditary hair loss, certain hair follicles are sensitive to dihydrotestosterone, or DHT for short.

Under the influence of DHT, these hair follicles gradually shrink. As a result, each growth cycle produces a finer, shorter, and less visible hair. This process is known as miniaturization.

In men, this usually follows a recognizable pattern:

  • the temples recede
  • the hairline gradually moves back
  • the crown thins out
  • the hair on top of the head loses volume

Hereditary hair loss typically develops gradually over months or years. Without treatment, the process often continues.

Stress-related vs. hereditary hair loss: how to spot the difference

The main difference is the pattern.

Is your hair suddenly thinning all over your scalp, and was there a clear trigger a few months ago? This is more likely to be telogen effluvium.

Is the thinning occurring gradually, primarily at your temples, hairline, crown, or the top of your head? This is more likely to be hereditary hair loss.

Can both forms occur at the same time?

Yes. Telogen effluvium and hereditary hair loss can occur simultaneously.

A stressful period can cause temporary, increased hair loss. As a result, hereditary hair loss that was already present may suddenly become more noticeable. In this case, stress is not the cause of the hereditary pattern, but it does make it visible sooner.

Hair may also appear to recover less fully after temporary shedding if hereditary miniaturization is also present.

Can stress accelerate hereditary hair loss?

Stress does not cause hereditary hair loss. That predisposition is already present.

A period of high stress can, however, cause more hair to fall out temporarily. This can make an existing receding hairline or thinning crown become visible more quickly.

When the diffuse shedding recovers, the hereditary pattern may remain visible.

What can you do about hair loss caused by stress?

For telogen effluvium, treatment with hair medication is usually not immediately necessary. The most important thing is to address the underlying cause and give the body time to recover.

Ensure you get enough sleep, eat a varied diet, and allow for recovery after illness or prolonged strain. Supplements are only useful if there is an actual deficiency.

See your GP if:

  • the hair loss lasts longer than six months
  • you are losing a lot of hair without a clear reason
  • you are very tired or have other physical complaints
  • your scalp is red, painful, or flaky
  • round or sharply defined bald patches appear

A GP can perform blood tests if necessary to rule out other causes.

What can you do about hereditary hair loss?

Hereditary hair loss usually does not resolve on its own, but it can often be slowed down.

The best-researched treatments for men are finasteride and minoxidil. Finasteride reduces the influence of DHT. Minoxidil stimulates hair growth and can support weakened hair.

Which treatment is suitable depends on your medical situation. A doctor will assess whether a treatment is safe and appropriate for you.
